STEP 2- IN FORM INCLUDE YOUR ORDER NUMBER RECEIVED WHEN YOU MADE YOUR PURCHASE. YOU SHOULD HAVE RECEIVED IT IN A EMAIL AFTER PURCHASE. (IF YOU DON’T KNOW YOUR ORDER NUMBER INCLUDE THE EMAIL YOU USED WHEN YOU MADE THE PURCHASE)

STEP 3 – SUBMIT FORM AND YOUR MEMBERSHIP WILL BE CANCELED FOR FUTURE RECURRING PAYMENTS.
